Must be member of Health Plan of San Mateo to be eligible for referral to MHA Community Support Services

Experiencing SMI/SUD
Aims to assist vulnerable population often ineligible for other services

Housing at Risk
Must demonstrate a clear need for services by demonstrating at risk status (i.e. lease violations, letter from landlord, photos of home)

Apply for HPSM Community Supports (CS)
Members who want CS can call HPSM Member Services at 650-616-2133.

Have your Primary Care, Mental Health provider, social worker or case manager complete and fax the following forms to:

Once approved, Community Support Case Manager will reach out and submit intake form to ARISE team. You can call the following to follow-up and expedite the process.
